As the Centers for Medicare & Medicaid Services (CMS) eliminates the Inpatient-Only (IPO) List, preventable surgical denials are expected to rise due to upstream coding errors.

The 2026 Final Rule shifts more procedures to outpatient and ambulatory surgery center (ASC) settings, increasing the risk of site-of-service conflicts, missing laterality, and other coding misalignments that drive claims denials.
